BIRTH CERTIFICATES

Congratulations on the birth of your baby!

Bluewater births are now registered at the Office of the Registrar General. A Statement of Live Birth can be completed electronically or in paper form and sent directly to the Office of the Registrar General. 

To obtain more information on birth registrations or to submit your birth registration electronically visit http://www.serviceontario.ca/newborn.  You may also apply for your child's birth certificate and Social Insurance Number using this electronic registration service.

DEATH CERTIFICATES

Requests for a Death Certificate for a death occurring in Ontario can be made by applying online at Service Canada, or obtain a paper form in person from the Municipality of Bluewater, 14 Mill Ave., Zurich, ON N0M 2T0.

All requests must be forwarded to the Office of the Registrar General at the address below.
